Every student who wishes to participate in a Good Samaritan youth event must submit a 2009-2010 health form. This health form covers ALL youth group gatherings, activities, retreats, trips, etc. for the entire school year. Forms can be mailed or dropped off at the church office attention Jean Elliott.
